This Salary Reduction Agreement ("Agreement") is entered into between the above-described employee ("Employee") of the University of Indianapolis and the University of Indianapolis ("University") in order for salary reduction contributions to be made to the University of Indianapolis Defined Contribution Retirement Plan ("Plan") under Internal Revenue Code Section 403(b). Participation in the Plan and this Agreement are subject to the terms of the Plan, as amended from time to time.
