This form ensures that equipment supplied by (school name) will be used and maintained in its original condition, reasonable wear and tear excepted, by the designated student . Students agree to demonstrate reasonable care with the respect to the security and physical well-being of the equipment signed to his/her care. In the absence of reasonable care or if the equipment is returned in a condition different than the original condition, parents will be financially responsible. In addition, in the event that (school name) determines in its sole discretion that the student did not utilize reasonable care in the use of the equipment, the parent will be responsible for any damaged, lost, or stolen equipment.
