By signing below, I understand and take responsibility to maintain and care for the rental instrument and case to the best of my ability. I understand that renting an instrument at a much reduced rate is a privilege afforded me through the purchase of instruments by the Sparkman High School Band Boosters. I have noted the current condition of the instrument above and will, to the utmost of my ability, maintain and care for the instrument and return it in the same condition as noted at the time of rental.
