i, , undersigned is the Chief instructor of the above dojang. I would like to apply to be a member of Chan Hun Ryu ITF TaeKwon-Do Federation and as an affiliated dojang. In consideration of my application to be a member in your federation and in participation in the program as well as events, activities, competitions faciliated, sanctioned or organized by Chan Hun Ryu ITF TaeKwon-Do Federation, the applicant (as well all dojang's members that including in the case of a minor child- under 18 years of ages-, the parent or legal guardian, hereby freely, voluntarily and without duress executes this agreement under the terms below:
