This Consulting Agreement (this “Agreement”) is made on February 1, 2023 (the “Effective Date”) by and between Tulare Local Health Care District, a Healthcare District formed under the California Health and Safety Code (“District”), and Teresa Jacques, an individual (“Consultant”), each a “Party” and collectively the “Parties”.
