For new enrollments, separate 403(b) enrollment applications must be requested from the District-approved vendor(s) you have chosen. This list is available at the District Office. Please return a copy of your completed enrollment application(s) to the District Office along with this salary reduction agreement form. The employee will need to send the original enrollment application information directly to the vendor to establish his/her accounts.
