This Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement (“Release”) is made and provided by the person signing below (“Participant”), who is being permitted to enter the fishing Vessel or utility vehicle (depending on whether summer fishing or ice fishing) (“Vessel”) owned by the fishing guide identified below (“Owner”) to participate in a guided fishing event (“Event”) on Devils Lake in North Dakota. Participant acknowledges and agrees that entering into this Release is required as a condition to entering and/or using the Vessel and participating in the Event. This Release shall be effective on the date of its execution and delivery by Participant.
