This Clinical Trial Agreement (this “Agreement”) is made as of 15 January 2021 (the “Effective Date”) by and between Oblastní nemocnice Kolín a.s., nemocnice Středočeského kraje with a place of business at Žižkova 146, 280 02 Kolín II, and any and all satellite facilities and/or affiliates of the aforementioned (collectively, “Institution”) and Paragon Biomedical Limited, a wholly owned subsidiary of Clinipace, Inc., with a registered address at Third Floor One London Square, Cross Lanes, Guildford, Surrey, GU 1 1UN, United Kingdom and a business address at Aston Court, Kingsmead Business Park, Frederick Place, High Wycombe, Buckinghamshire, HP11 1JU, United Kingdom (together with its affiliates, hereinafter referred to as “CRO”).
