Enclosed is the Dental Plan Group Agreement effective January 1, 2013 through December 31, 2013 for City of Vancouver. There are two documents which serve as the entire contract. The Wrap (titled “Group Agreement”) contains the group contract provisions, including rates. The Evidence of Coverage (EOC) is the member portion of the contract. The EOC contains benefit descriptions, limitations, exclusions, and instructions which assist the member in obtaining care. Wraps and EOCs are subgroup specific and it is possible to have multiple Wraps associated with the same EOC. Please review the list of contract changes including those things that may have been updated since the renewal notice was released.
