Welcome and thank you for considering me in the treatment of your mental health needs. I realize that starting counseling is a major decision and you may have many questions. This document (referred to as the Agreement) is intended to inform you of my policies, state and federal laws and your rights as a client. Please read it carefully and jot down any questions you may have so that those questions may be discussed at your first counseling session. When you sign the Acknowledgment of Informed Consent, it will represent an agreement between us that you are consenting to the terms included in this Agreement. This means you will be agreeing that you have read and understood what is contained in this document and agree to be bound by all the terms, conditions, and information it contains so please read over it carefully and we can discuss any questions you might have when we meet for your session.
