AMENDMENT TO AGREEMENTBETWEEN THE COUNTY OF SAN MATEO AND CHILDREN’S HEALTH COUNCIL THIS AMENDMENT TO THE AGREEMENT, entered into this day of , 20 , by and between the COUNTY OF SAN MATEO, hereinafter called "County," and CHILDREN’S HEALTH COUNCIL, hereinafter called "Contractor"; W I T N E S S E T H: WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof; WHEREAS, the parties entered into an original Agreement for professional services on December 14, 2004; and WHEREAS, on December 13, 2005 the original Agreement was amended, extending the term and increasing the Agreement maximum; and WHEREAS, the parties wish to further amend the Agreement to increase the Agreement maximum for the period of June 1, 2006 through June 30, 2006. NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS: 1. Paragraph 3. Payments is hereby deleted and replaced with
