Mandated Data Processing Agreement Title Whole Systems Integrated Care Related ISS Whole Systems Integrated Care Summary Description The WSIC System links provider data from health and social care organisation to generate a summary integrated care record stored in a secure dedicated data warehouse. This summary record can be viewed by the health and care professionals involved in a patient’s treatment, thereby providing improved clinical decision making. The system is also used to provide population health management, proactive care and optimised care management. The longitudinal record allows the review of patient pathways, improving transitions of care, and removing inefficiencies in the delivery of care. There is a fully de-identified version of the WSIC data (ISB 1523) in a separate secure data warehouse that is used to support translational research for healthcare delivery and planning.
