This membership agreement (this “Agreement”) is entered into by and between THE CENTER FOR CARE INNOVATIONS, a fiscally sponsored project of the Tides Center, a California 501(c)3 not-for-profit (“CCI”, “we”, “us” or “our”) and any individual or entity who accepts this Agreement (“you”, “your”, “Applicant”) and is made as of the date of such Applicant’s acceptance of the terms of this Agreement.)
