This Agreement and Declaration of Trust of the Producers' Health Benefits Plan (herein the "Plan" or "PHBP") originally made and entered into the 1st day of January, 2001 (under the name "AICP Health Benefits Fund"), as Amended and Restated as of September 29, 2015, November 29, 2018, January 1, 2020 and January 1, 2022 and is further Amended and Restated effective January 1, 2023 by the undersigned Trustees.
