This agreement is entered into by and between the Bellefontaine Finance and Development Authority dba PeakFiber, an Ohio Port Authority, with offices at 100 S. Main St., Bellefontaine Ohio 43311 (“PeakFiber”) and you (“Customer”) for residential broadband fiber service. Both parties agree to the terms and conditions provided within this document and any other supplemental documents that may be referenced.
