This Restricted Data Use Agreement (“Agreement”) is by and between The Regents of the University of Michigan, a Michigan Constitutional corporation with its principal place of business in Ann Arbor, Michigan (“Michigan”) and (“User”) and is effective on (the “Effective Date”)
