This Software as a Service Terms of Service Agreement (the “Agreement”) is entered into between JourneyLabs Inc., a Delaware Corporation with its principal place of business located at 1511 E SR 434, Suite 2001, Winter Springs, FL 32708 (the “Company”) and the Clinician, as described in the Quotation Sheet, incorporated by reference herein, and will be effective from the date specified on the Quotation Sheet (the “Effective Date”). (Company and Organization may be referred to singularly as a “Party” or collectively as the “Parties.”)
