This Agreement (hereinafter referred to as “Agreement” or “Contract”) is entered into between the State of Connecticut (hereinafter “STATE”) acting through the Office of Health Strategy (hereinafter “OHS”) pursuant to C.G.S. §19a-754a,C.G.S. § 4-8 and Special Act 21-15, and Anthem Health Plans, Inc., doing business as Anthem Blue Cross and Blue Shield (hereinafter “CONTRACTOR”), a company having its principal offices at 108 Leigus Road, Wallingford, CT 06492 CONTRACTOR (collectively the “Parties”). The Parties agree that the services specified below shall be provided by CONTRACTOR in strict compliance with the provisions of this Agreement.
