This Client Service Agreement (hereinafter the “Agreement”) is between HomeHealth Advantage, Inc., a Utah Corporation (hereinafter the “Company”) and Bladen County Home Health Agency, located at 300 Mercer Road in Elizabethtown, NC (hereinafter the "Client”), effective the date as set forth in Subparagraph 3 below, entitled “Length of Agreement and Potential Amendments.”
