By submitting the sponsor/exhibitor registration form, I authorize WAPF to charge the appropriate fees. I understand all cancellations must be submitted in writing and must be received by September 12, 2016 to be eligible for a refund of 50%. After September 12, 2016, no refunds will be issued. I acknowledge that I have read the complete exhibitor kit which provides detailed listing of fees associated with shipping, electric, internet access, etc. Please note: you must sign and date page 2 of this registration form. All sponsor/exhibitors must be approved by the WAPF Board.
