THIS HEALTH SERVICES AGREEMENT (this “Agreement”) is made and entered into to be effective as of August 8, 2014 (the “Effective Date”) by and between CITY OF FORT COLLINS, a home rule municipality (“Client”), with principal offices at PO Box 580, Fort Collins, CO 80522 and MARATHON HEALTH, INC. (“Marathon”), a Delaware Corporation with principal offices at Champlain Mill, 20 Winooski Falls Way, Suite 400, Winooski, VT 05404. Client and Marathon may each be referred to in this Agreement as a “Party” and, jointly, as the “Parties”.
