The Department of State Health Services (“System Agency” or “DSHS”), an administrative agency within the executive branch of the state of Texas, and CHOLAJ, LLC (“Contractor”), having its principal office at 11227 Balmullo CT Richmond TX 77407 (each a “Party” and collectively the “Parties”) enter into the following agreement (“Contract”) to provide Coronavirus Disease 2019 (COVID-19) vaccination services through a Vaccine Administration Team(s) (VAT(s)) to residents living in the State of Texas.
