CareerSource Flagler Volusia, (hereinafter CareerSourceFV) a Florida private not-for- profit corporation, whose principal address is 329 Bill France Boulevard, Daytona Beach, Florida 32114 and Case Management, Inc. (hereinafter Contractor), whose principal address is 719 Tumblebrook Drive, Port Orange, Florida 32127.
