I, (either individually or as representative of group) do hereby agree to printed Policies and Procedures and fully release and discharge First Baptist Church Clinton from all liability of any kind and character upon any claim, demand or cause of action which might be asserted on behalf of the above individual or group against the First Baptist Church, Clinton Mississippi.
