This CyberGRX Vendor Assessment Agreement (“Agreement”) is between CyberGRX, Inc., a Delaware corporation, with its primary place of business at 1637 Wazee Street, Suite 400, Denver, Colorado 80202 (“CyberGRX”), and the company identified on the signature page (“Vendor”). This Agreement is effective as of the date of last signature (“Effective Date”). CyberGRX and Vendor are each a “Party” and collectively the “Parties.”
