This DATA USE AND NON-DISCLOSURE AGREEMENT (“Agreement”), effective on the date specified on the Signature Page (“Effective Date”), is hereby entered by and between the New York City Department of Homeless Services (“DHS” or “Data Owner”), having its primary offices at 33 Beaver Street, New York, NY 10004, and New York City Department of Health and Mental Hygiene (“DOHMH” or “Recipient”), having its primary offices at Gotham Center, 42-09 28th Street, Queens, NY 11101, (each a “Party” and, collectively, the “Parties”).
