INDIAN CAPITAL TECHNOLOGY CENTER BOARD OF EDUCATION EFBC-R1 Student Device Loan Agreement EQUIPMENT ISSUED (“Equipment”) Indian Capital Technology Center, ICTC, (“School District”) has issued/loaned certain School District equipment, more particularly described above (“Equipment”), to Student and Student’s Parent/Legal Guardian (collectively “Borrower”), for educational purposes during the academic school year. By initialing, School District Equipment Agent represents that the above Equipment has been issued to Borrower in the condition indicated above, and if returned, returned in the condition indicated above. ACKNOWLEDGMENT OF RESPONSIBILITIES Borrower acknowledges and agrees that Borrower’s use of the Equipment is a privilege. By entering into this Agreement, Borrower acknowledges that it is Borrower’s responsibility to protect and safeguard the Equipment, to ensure that it is kept in a safe environment, and to return the Equipment in the same good condition and repair.
