The following are the basic terms of the Licensing Agreement (the “Agreement”) between Creative Life Systems Ltd., with an address at 6 Court Lodge, Lamberhurst, Kent, TN3 8DU (“Distributor”), and the undersigned (“Licensee”), in connection with the licensing of the documentary motion picture entitled Orgasmic Birth (the “Picture”).
