Cable/Internet 1 Year Agreement InstallationCable/Internet Agreement • October 29th, 2012
This agreement sets forth the terms and conditions fo rIPTV/Internet service provided to the subscriber. Upon acceptance of these Terms and Conditions SMTA will provide installation of service, the first TV is included in the installation price. If additional TV's are requested after the initial installation an additional charge will apply. An early termination fee of $200 will apply if services are terminated prior to one (1) year under contract. Set top boxes and DVR's leased from SMTA as a part of this agreement remain the property of SMTA. Damage to the set top boxes leased to the subscriber casused by abuse, lightning, fire, or other natural or man-made acts is the responsibility of the subscriber. An Equipment Protection Plan may be purchased to cover damage not caused by abuse.
