This Placement Services Agreement (“Agreement”) is entered into as of this day of , 201 (“Effective Date”) between DeKalb Medical Center, Inc. (“DeKalb Medical”) and (“Company”) regarding the placement services Company may provide to DeKalb Medical and the fees that are payable by DeKalb Medical to Company following Company’s successful placement of a candidate with DeKalb Medical.
