I, the parent/guardian, realize there is a risk of personal injury and/or illness to the participant named on this application by his/her participation in the Boone Area Summer Swim Team activities and programs. I will not hold liable, the Boone Area Swim Team or the City of Boone for accidental injury or illness resulting from this participant’s attendance and activities associated with the Boone Area Summer Swim Team.
