AN AMENDMENT TO THE AGREEMENT BETWEEN THE COUNTY OF SAN MATEO ANDROSANNA KOWSKI THIS AMENDMENT TO THE AGREEMENT, entered into this day of , 20 , by and between the COUNTY OF SAN MATEO, hereinafter called "County," and ROSANNA KOWSKI, hereinafter called "Contractor"; W I T N E S S E T H: WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof; WHEREAS, it is necessary and desirable that Contractor be retained for the purpose of respite care. WHEREAS, the parties entered into an Agreement on November 27, 2006 wherein Rosanna Kowski agreed to provide respite care to youth served by Children and Family Services, Juvenile Probation and Mental Health clients for the term of October 15, 2006 through June 30, 2009 for a maximum payment amount of $27,114; and WHEREAS, the parties wish to amend the Agreement for Rosanna Kowski to continue providing respite care and to add
