I hereby permit Christ Lutheran and/or its agents to take my child/ren on field trips beyond the campus. I understand I will be given prior notification of such field trips. In the transport of my child to and from these activities, I release you from any and all liability in the event my child is injured during an accident associated with Christ Lutheran or its agents.
