This form, completed, endeavours to ensure the child/vulnerable adult is protected through the choice of having or not having their photo/video taken. The parent/child/vulnerable adult are required to sign this. By doing this it will be understood that the child/vulnerable adult has an understanding (as far as possible) to be able to say YES or NO to the photographer. This form will need to be signed once a year, 1st January – 31st December. Should there be concerns in between this period the Safeguarding Officer should be informed. The parent/child/vulnerable adult, between the stated period, have the right to change their mind.
