Counseling Disclosure Agreement • January 30th, 2021

This agreement serves as notice to the client that any advice given by the FBC is at the sole discretion of the client to implement. The preferred counselor’s advice and principles given during sessions are not required to be used. The preferred counselor is not in any shape or form liable for any advice given as the preferred counselor is only going on their expertise, biblical principles and the information relayed from the client during the sessions. The client(s) failure to disclosure all information or share from a one-sided perspective does not put the FBC at any risk for being liable. The client does hereby agree that they waive all legal rights should they implement any advice or act in any way that is not favorable to them after having a session with the FBC above.
