This Amendment No. 5 (“Amendment”), effective as of July 1, 2021, to Alternate Contract Source No. 72154066-ACS-15-1, Equipment Maintenance Management Services (“Contract”), is made by and between the Department of Management Services (“Department”), an agency of the State of Florida, located at 4050 Esplanade Way, Tallahassee, Florida 32399, and Specialty Underwriters LLC (“Contractor”) located at 9667 S. 20th Street, Oak Creek, Wisconsin 53154. The Department and Contractor are collectively referred to herein as the “Parties.”
