This Business Associate Agreement (“Agreement”) is entered into between the State of Utah (“STATE”) and (“BUSINESS ASSOCIATE”) for purposes of compliance with the Health Insurance Portability and Accountability Act of 1996, 45 C.F.R Parts 160, Part 162, and Part 164, as amended (“HIPAA”), and the Health Information Technology for Economic and Clinical Health Act, 45 C.F.R. Part 160, as amended (“HITECH Act”).
