The Honolulu Emergency Services Department, Emergency Medical Services Division (EMS) is the primary 9-1-1 pre-hospital emergency medical services operated by the City and County of Honolulu. Currently, the State of Hawaii, Department of Health Emergency Medical Services and Injury Prevention System Branch (State) manages all ambulance transport billing and collection. Recent statutory amendments transfer ambulance transport billing and collection services to the City effective July 1, 2022. The City does not have the capacity, experience, manpower/workforce or the expertise to manage ambulance transport billing and collection services on its own.
