I/we, , the undersigned, have legal custody of the student listed above, and have given our consent for him/her to attend events organized by Columbus Road Baptist Church (“CRBC”). I/we understand that there are inherent risks involved in any ministry or athletic event. I believe and represent that my/our child is healthy and physically able to participate safely in these activities. I/we agree to indemnify and hold harmless CRBC and its Deacons, employees, agents, volunteers, and/or officers from any liability arising from participation in the activity listed above. It is further acknowledged that any CRBC activity may involve transportation in a personal vehicle, a van, or a bus. The terms of this release shall be governed by the laws of the State of Illinois.
