This contract signed by you and your parent/guardian is required prior to purchasing a ticket and attending Branham High School dances. If you are bringing a guest, a signed contract from him/her and his/her parent is also required and can be found on the bottom of this sheet. A copy of the guest’s current ID and school assistant principal approval is also needed. There will be NO REFUNDS. Please review ALL information carefully before signing.
