An Agreement to Waive Reevaluation form (AWR) is completed when the student’s team determines that a reevaluation is not necessary. Typically, a reevaluation is conducted to determine continuing disability and need for special education services. The student’s parent or guardian must sign this form to confirm agreement (or disagreement) and acknowledge their right to initiate further action on the student’s behalf.
