This addendum supplements the Clinical Education Agreement, the AGREEMENT, between the University of Kentucky, the UNIVERSITY, and , dated 2020. It sets forth the terms and conditions upon which University of Kentucky, College of Social Work, hereinafter DIVISION, students may undertake clinical learning experiences at the facilities of AFFILIATE.
