Network End User License Agreement Between DataHEALTH and CustomerNetwork End User License Agreement • April 13th, 2011
This Network End User License Agreement (the "Agreement") is made between DataProtech, Inc., a Kentucky corporation doing business as DataHEALTH ("DataHEALTH"), and the applicable customer of Patterson Dental Supply, Inc.’s (“Patterson”) PATTLOCK™ data backup solution (“Customer”) who have previously executed the PATTLOCK™ Back-up Solution PATTLOCK™ Order, Terms & Conditions (“Order”). The Agreement contains the terms and conditions under which Customer can use DataHEALTH’s network and offsite data storage location and equipment using DataHEALTH’s network software and portals. This Agreement is further amended and supplemented by the HIPAA Business Associate Agreement (the “HIPAA Agreement”), attached hereto as Appendix A and made a part hereof as if fully incorporated by reference.
