It is agreed by and between the State of Vermont, Department of Vermont Health Access (hereafter called the “State”) and Blue Cross and Blue Shield of Vermont (hereafter called the “Contractor”) that the contract on the subject of personal services generally on the subject of offering qualified health and dental plans on the State of Vermont’s federally- mandated health exchange, effective August 30, 2013, is hereby amended effective January 1, 2016, as follows:‌
