RELEASE & HOLD HARMLESS AGREEMENT: By registering for and requesting any one of the following: herbal, sage, tea, or other traditional medicine items or kits, from the Cowlitz Indian Tribe, you and/or your family member guests, including minor children, acknowledge that there are unknown and/or inherent risks of injury, harm, or exposure to herbal medicines and/or other diseases through exposure to contaminated objects, as well as through personal contact, and through consuming, applying, or inhaling, herbal medicine. Such exposure carries with it a certain degree of risk that could result in allergic reaction, including anaphylaxis, illness, disability or death of you or your guests. You hereby agree to release and hold harmless the Cowlitz Indian Tribe, its employees, officers, directors, volunteers, and vendors from and against all claims of damages and liability resulting from injury, harm, or exposure to herbal medicine or other diseases of you or your family members using or cons
