This Contract (“Contract”) is entered into by and between the undersigned provider in the Colorado Medical Assistance Program (“Contractor”), and the STATE OF COLORADO acting by and through the Department of Health Care Policy and Financing, 1570 Grant Street, Denver, Colorado 80203 (“State” or “Department”). Contractor and the State hereby agree to the following terms and conditions:
