I, , (the CLIENT), engage American CryoStem Corporation, and its affiliated Laboratory Processing Facilities and Cryopreservation Facilities, hereafter collectively “ACS” to process, cryopreserve and store stem cells obtained from adipose tissue collected by your surgeon/doctor on the following terms (this Agreement).
