For and in consideration of CPI Holdings LLC, DBA -Breakers Girls Basketball or Breakers Elite Boys Basketball allowing the minor(s) identified below (individually and collectively, “Minor”) to participate in any Club sponsored, organized and/or supervised events and/or activities (the “Activities”); I, for myself, and on behalf of Minor, and the Minor’s other parents/legal guardians, heirs and next of kin, and any legal and personal representatives, executors, administrators, successors and assigns, hereby agree to and make the following contractual representations pursuant to this Waiver and Release of Liability, Assumption of Risk and Indemnity Agreement (the “Agreement”);
