This Fourth Amendment to Agreement (“Fourth Amendment”) is made by the County of Alameda (“County”) and Kaiser Permanente On-the-Job (“Contractor”) with respect to that certain agreement entered by them on October 18, 2018 and that certain First (June 2, 2021), Second (June 30, 2022), and Third (June 15, 2023) Amendment(s) to Contract, (collectively referred to herein as the “Contract”) pursuant to which Contractor provides pre-employment physical examinations and occupational health services to County.
