This Addendum is entered into by and between the provider, duly qualified and licensed by the State of Idaho, and the PQA, for participation in the Blue Cross if Idaho (BCI) ConnectedCare Products. Blue Cross of Idaho Health Service, Inc., is a mutual insurance company, and is hereinafter referred to as “BCI.”
